There are two routes to obtaining My ID Card and they are followed at all times:
Route 1 is for people who already have at least one of the following documents from LIST A on the application form. This includes: birth certificate, passport, driving licence, medical card, GP registration letter, or UK armed forces card. A verifier should sign a photocopy of one or two of these documents, and a photocopy of one of the documents from LIST B on the application form (e.g. college letter, utility bill).
A verifier for route 1 should be someone like a Director of a Limited Company, Doctor, Lecturer, or Teacher. This verifier also needs to complete and sign the application form and one of the applicant’s passport photos.
Route 2 is for people who DO NOT have a passport, birth certificate or driving licence. A verifier must certify that the name, date of birth, age, gender and home address on the application form matches the records held at their organization e.g. school, medical practice, registry office.
A route 2 verifier must have one of the following professions: teacher, tutor, head of year, administrator at the applicant’s school, college or university, doctor, nurse, dentist, social worker, certified solicitor, a bank or building society staff member at a branch, job centre staff member, DWO advisor, or a prison or probation officer. The verifier also need to complete and sign the application form and sign one of the applicant’s passport photos.
